The general rule for multimedia citations is to include as much information as you have available to you. Note in the example the inclusion of original publication information and format as well as the publisher and format the researcher found the item in.

General Format 
Full Note:

1. Author First Name and/or Initial Surname, Multimedia Title (Place of Publication: Publisher, Year), Format, URL.

Concise Note: 

1. Author Surname, Multimedia Title.
